Terms of Service |
The application to obtain a smart card is to be submitted by the personal presence of the concerned person. |
Proof of identity for the applicant for first time issuance, and the testimony of an adult individual of the family |
Required Documents to Obtain the Service |
1. ID card application form. |
2. Proof of identity for the applicant or a first-degree identifier. |
3. The consent of the guardian for those under 16 years old. |
4. Recent color personal photos (2). |
5. A letter from the General Command for military officers with the approval to issue the card. |
6. Bridge Statistics Card for those who need it (the original + a copy) or the seal of the Follow-up and Inspection Department. |
7. Medical test for blood type or a written pledge from the concerned person. |
Service Procedures |
1. Fill in the Smart ID card application and take a number by the Public Service Office. |
2. Pay legal fees to the Treasurer. |
3. Match the information with the application and accept them according to procedure by the Document Acceptance Officer, (check for non-existence of records): |
- Enter the financial receipt - Iris of the eyes photography |
- Enter blood type - Declared address |
4. Approve the application by the Office Secretary or his assistance. |
5. Refer the application to the Archiving Section, to archive the document. |
6. Refer the applicant to the Registration Officer (take finger prints) and signature. |
7. Send the print order electronically to printing centers. |
8. Send a text message to the citizen informing him the card is ready to deliver. |
9. Verify citizen’ data through entering the fingerprints. |
10. Enter the card’s password for the citizen. |
11. Hand over the smart ID Status card to the service recipient. |
