The final date for approval of the old ID card at public institutions and departments is 31 /3/2018.

When applying for an ID for the first time, and issuing a replacement for a lost card.

Temporary passport / residence visa of head of the family for Gazans/ card for children of Jordanian women, if any + a copy.

•  The original marriage contract if the marriage took place outside the Kingdom duly certified or a marriage certificate from the Civil Status Department if the marriage took place inside the Kingdom for each wife + a copy.
• A copy of the document of the wife (passport/ residence card/ card for children of Jordanian women) 
• Copies of the children’s documents (passport / residence card / cards of children of Jordanian women / Jordanian birth certificate / birth certificate of a foreign country).

• The application should be submitted by head of the family, or the wife in justified cases that are approved by the Office Secretary.

• The Family Statement includes wives regardless of their nationality, and single children only.

• A divorcee, from the Gaza Strip, can submit an application for Family Statement without adding the children, on condition that she doesn’t have another nationality.

• A widow of a Gazan man can submit an application to register a Family Statement for her and her children.

• Gazan women who are married to Jordanians or holders of a foreign nationality cannot file an application for a Family Statement
