| Terms of Service |
1. Applicants who are 18 years old and above should submit the application in person. |
| 2. Mother, father, or guardians should submit the documents for minor children. |
| 3. Acknowledgment from the applicant, stating that the children are inside the Kingdom, if the cards are to be renewed are for children. |
| 4. The personal number is approved for holders of temporary passports from the West Bank and Gaza Strip, according to numbers issued from the Department. |
| Required Documents to Obtain the Service |
1. The card to be renewed. |
| 2. Color personal photos (2). |
| 3. Passport/ residence card for a foreigner with a personal number written on it. |
| 4. A financial pledge to preserve the document for the value of JD10 + 250-piaster stamps. |
| 5. Notification from the police station of the loss. |
| Service Procedures |
| 1. Fill in an application form to obtain the service and get a number by the Public Service Office. |
| 2. Match information with the application and enter the financial guarantee by the Document Acceptance Officer. |
| 3. Approve the application by the Office Secretary or his assistance. |
| 4. Pay legal fees to the Treasurer. |
| 5. Refer the application to the Archiving Section, to archive the application. |
| 6. Refer the application to the Printing Section, to print the card. |
| 7. Hand over the card to the service recipient after handing the financial receipt to the Delivery Officer. |
